Sam has been struggling with depression since Stanford. Well, not really struggling. Excelling. He's been excelling at depression since Stanford, and it's only gotten worse since he started hunting again. Unfortunately, the relapses from John's death and Dean's trip to Hell were bad, but they had nothing on the downward spiral triggered by Sam starting the Apocalypse.
But Sam has never come clean before, and he isn't about to start. He's self-medicating, he's suppressing, and he's handling things in the Trademark Winchester Way. He can do this. He can totally do this.

Dean finishes his prayer. The room is washed ocher by the glowing red sign outside; the navy blue bed sheets are a bloodied near-black. It’s an eyesore, dominating the senses, so Dean looks instead to the grey night sky alight with the light pollution and adorned with cotton balls clouds that have been disembodied by the atmosphere. Dean blows out his own cloud through the motel’s screen window to accompany them, courtesy of the Marlboro Black he has perched on his bottom lip with its filter going brown between his teeth. He made a bad choice with this pack, he muses, curling his tongue around the morgue flavor in his mouth. It’s not worth the familiar wheeze his lungs sing in protest, but he stains and burns his shaking fingers lighting one after another and putting them out in the sink, until the cigarettes bring about the migraine they were supposed to cure.

Too bad he didn't tell anyone/didn't realize he was already coming down with something before then. How long will it take Dean and Bobby to realize that there's more to Sam's fever and chills than just withdrawal, and that maybe Sam needs some help in there?

Summary
Every part of Sam burns. There's blood on his lips that's not his own (not that kind; it doesn't taste right), and even the smallest rustle or crunch of leaves crashes against his eardrums and sends pain splintering through his skull. He's so thirsty.
Frantic attempts to remember how he got here leave him with a soup of images and impressions—color, tension, blood, pain, desperation. He starts from what he knows. He was on a hunt...
In other words... Still reeling from what happened when Sam was soulless, Bobby gets careless and a vampire turns Sam before they can put it down. Badly wounded, Sam's body uses all its remaining resources in an attempt to slowly heal him, and it's a long way back home. The cravings and even the agony of the cure aren't the worst things Sam has to face.

Lucifer was cradling his face and it sent fear through every part of his mind that was still aware. He was speaking gently, but his words were cruel.
“Why do you think Dean didn't kill you?”
“I don't know” Sam answered honestly, because what was the point of lying anymore? He had nothing left to give, he had nothing left to hide. It was cold. So, so cold. Sam hated the way that he pressed more into Lucifer's hands because somehow they were warm, warm despite the cold.

Tucked away in the bathroom that morning, Sam resolved he still couldn’t fully recognize himself. He practiced a smile in the mirror, squinted his eyes and scrunched his nose, the way he naturally used to.
With everything stripped away, no sense of self, Sam realized that there was nothing else left inside.
