She looked older. Taiyang supposed he'd never really gotten a good look at Ruby since the accident. The hospital had given her so many creams and bandages he wouldn't have been able to see much if he tried, but it was his lack of trying that really let him down this time. To be honest, he hadn't wanted to see what she looked like after. Not because he didn't want to see her burns, but because he didn't want to see her in pain he couldn't do anything about.
When he looked into her eyes, at her face grinning up at him, he saw she was more than happy to see him. But she was also in pain.
That's why she looked older.

Summary
Izuku is in the middle of a fight against Shigaraki Tomura, trying to save the boy named Tenko deeply hidden within Shigaraki Tomura's mind. In a bid to save the child deep within, he is ultimately killed for his trouble inside the mindscape instead of Tenko at All for One's hand.
Due to his vestige being killed in the Vestige world, All for One takes control over Izuku's body. Leaving Shigaraki Tomura to fight his mentor all on his own without Izuku being present, and even though Izuku's vestige is no longer present the boy's body makes it impossible for All for One to survive a fight against his successor.
