“Better now?” said Dream’s voice, and Tommy nodded shakily, even though the player he’d glimpsed a moment ago was definitely not Dream.
Not-Dream helped him to his feet and guided him gently away from the puddle of sick. Tommy managed a few steps before he had to sink to his knees and catch his breath.

In the aftermath of exile, Tommy starts to understand the bigger picture. Somehow, it’s far worse than he had ever imagined. And Tommy can imagine quite a lot (or maybe it wasn’t his imagination after all).

As soon as Tommy wakes up, he knows something is wrong. His throats feels scratchy, his skin sticky with sweat, and his eyes feel crusted shut. He stumbles onto his feet, ignoring the aching pulling down on his body with every moment. A shiver runs through him as he drags himself towards the bathroom, fingers trembling around the doorknob.
Staring into the mirror, he almost doesn't recognize himself.
His hair is a mess, curls sticking to his forehead. There's a flush in his cheeks and a sag in his overall posture.
"I look like shit," Tommy mumbles to himself.

In the times Dream lives in prison, he comes up with a plan. He really wanted to get Tommy for his plans, but it would be quite hard because Tommy knew a lot of his tricks and was harder to manipulate. So Dream gets a great idea, he would travel back in time to the first L'Manburg war, where Tommy was young and naive. The twelve-year-old boy would be way easier to sway to Dream's side.
So with the help of some 'friends' and a God, Dream successfully creates a time machine. Going back in time to a timeline where the Independence War was just happening, Dream starts his plan. Wilbur is the President, Tommy is a child. They aren't ready for what Dream got planned.

But he remembered the trick the poet had written about—how you know you’re alive as long as you can still make a fist. Tommy as a child had giggled and slugged his brother in the shoulder, but Tommy today had nobody left to fight. He didn’t think he had the strength to fight anyone anyway, but still. He’d become so familiar with his anger, his rage like a spitting wildcat in his throat, in his fists; his fear was a constant companion, too. Not even in the latter stages of his exile had he felt so… empty.
But he could still make a fist.
