“Booooo! No, you know the rules! No seriousness on Lazy Sundays!” Mikey is a big proprietor of lazy days and makes sure everyone follows the rules of having a stress free day. It works only because it guilts them all to rest, if only so Mikey isn’t stressing about them not resting.
“It’s Thursday Micheal.”
“Yeah but it has the spirit of Lazy Sunday”
“Guys!” Raph’s outburst brings their attention back to him, but their deadpan stares show they still don’t appreciate the seriousness Raph is trying to infringe upon their relaxation.
“Guys, we got a code Sleepy Slider on our hands.” Raph’s voice is grim, his face shadowed with the weight of the situation they find themselves in.
With the code spoken, Donnie and Mikey sit at attention.

You would think, after drinking an antidote for a mystic poison that his stupid siblings had purposefully ingested, that it would cure them of their illnesses and ailments. But Leo suddenly sways and falls onto the ground.
“Leo!” Panic had stricken Raph then, almost making him lose control. The slider’s skin had been hot to the touch when Raph had picked him up.
Looking up at his remaining brothers, he had noticed they also did not look as healthy as they had been half an hour ago – Donnie was clutching his stomach, breathing heavily, and Mikey was also huffing sharp breaths of pain and his limbs shook.~
Though the brothers managed to take the antidote for the poisoned Pizza Puffs, they are still suffering the aftermath of the poison and it’s symptoms. Raph takes care of them, because that’s his duty as the oldest brother.
TLDR: Leo, Mikey and Donnie are still sick after eating mystic poison. Raph takes care of them.

That is the aftermath of fighting The End. Sage is dead, by Sonic's own fault, and it's a guilt he cannot live with. He has to fix it. Even if fixing it means throwing a copy of his memories into the past in hopes that he can learn from his own mistakes.
It's one island. One fight. That's all he has to do. He just has to do it right, and he'll have saved Sage. And he can pretend like nothing ever happened, and like he was the perfect hero Sonic the Hedgehog is supposed to be all along.
