Summary
McCoy has, on many occasions, sat down and calculated the exact number of punches thrown by a being of average humanoid weight and strength that each of his closest crewmembers could take. A morbid thought exercise, to be sure, but a necessary one. The crew of the U.S.S. Enterprise refuse to acknowledge their own limits, so McCoy has to figure it out for them, if just so he knows exactly who to treat first should a seriously nasty incident occur.
Of course, he declines to share this particular hypothetical with anyone. If Kirk or Spock caught wind of it, they'd become insufferable. Kirk would no doubt try to beat McCoy's estimate, and Spock would probably come up with some petty, prideful excuse hidden beneath twenty layers of unflappable Vulcan logic as to why he should actually be the one most likely to come out on top.
Now, though, in this dark, tomb-silent room, McCoy begins to wonder, for the very first time, just how much he can handle.

Summary
While attempting to infiltrate and disband Atillius, one of the galaxy's most brutal and notorious alien gladiator rings, Spock and McCoy are taken and trafficked into the organization.
Kirk and the rest of the crew manage to rescue them months later, but when they do, they find that neither of them are the same anymore.

Jim thought it wasn’t hard to understand at all. It was quite simple when you looked at it this way; Spock, forced to fight to the death for his and McCoy's life day after day, had lost his mind in the process. He'd become Atillius's perfect killing machine, and in the end, it had driven him insane."

Summary
Doctor McCoy sure as hell doesn’t compromise care, confidentiality, or his goddamn conscience for anyone. He has no problem with extracting a mind-controlling slug from Uhura's sinuses, performing a blood transfusion between a human and a sentient puddle, or administering therapy to a crew-wide hallucination of a god.
But catching feelings for Spock?
Now that’s just irresponsible.
And ultimately, it's all Jim’s fault.
