Summary
It was a well known fact that at the beginning of the Enterprise’s journey into deep space Leonard McCoy hated formal diplomatic dinners.
Most of the Enterprise crew thought it was because of the scratchy and stiff dress uniform that they get stuffed into for these events. While that was certainly a contributing factor to his hatred, he wouldn’t say it was the only reason.

Leonard hates diplomatic schmoozing. Not only does it feel inauthentic, but he also has to put up with everyone fawning over his brilliant boyfriend. (Not that he could blame them.) During his distracted musings he misses something that annoys Spock and causes him to wield the sharpest Southern phrase: "Well, bless your heart, Ambassador." Leonard chokes on his drink and gives them a great excuse to leave, but Spock misinterprets the laughter that follows when they're alone.
Based on tygermama's (Tumblr) scenario of Leonard overhearing Spock say “Well, bless your heart!” correctly.

Summary
A successful accelerator physicist comes to the Health Center with a minor electrical injury. By the time he leaves, the doctor has a crush. By the time he begins commissioning a new ion collider and becomes too busy for dating, that doctor is in too deep to back out.
The doctor might be a tiny bit jealous of that collider.
