Both wore thick leather collars, attached to black steel leashes that glinted under the light of the full moon. III was nearly unrecognizable to the guitarist; he wasn't sure what the short man had whispered in III's ear, but he had been forced to watch when III had dropped to the ground and began growling, lunging towards IV like a rabid dog. The only thing that had kept III from getting to the smaller man was the leash, rattling with every pull of III's body against it. Vessel had watched in horror as well, wondering if the man holding his leash would turn him into some mindless animal as well."

III squeezed his hand once more before letting go, presenting his coffee to him with two hands and bowing his head as if offering him a precious gift. “It was lovely to meet you. Hopefully we see you around more often.”
He graciously accepted his drink, tilting the cup towards III in a silent thank you, turning towards the door to continue his journey through town, getting to know the area he was suddenly a bit more comfortable calling home.

It's not III's job to explain the physiology to the families. Sometimes, the families don't even know why III is there. But as they cry and say the things they wished to say before their loved one had died, III is present, passing on the messages, and sometimes, passing some back.
Just one more word. One more I love you. III ignores the requests if they're not centered in love. They aren't being paid pennies to ask the recently departed to uncover the families secrets - they are there to soothe and support the transition. They're there to speak when the brain and body is holding on but the soul has decoupled from the physical. He holds their hands or their bodies close in the spirit realm, watching light come back to their eyes when they realize that it doesn't hurt anymore.
There is no such thing as pain in the spirit realm. It's a mortal concept. III stays as long as they need. And when they are ready, if they are ready, they lead the soul into the gentle hands of Death's reapers.
