What would’ve happened if one of those children had survived?"

It’s not fair, Astarion finds himself thinking as he stares intently at Gale’s stupidly elegant hands, the professor gesticulating animatedly while talking about… something or other that Astarion could not bring himself to care about, let alone focus on, that he looks so goddamned hot explaining things.
Or, Astarion has a thing for his friend and coworker's hands, as well as his overall competence, and things develop from there.

“Are you alright?” Astarion asked in concern, stepping to go to Gale’s side, to comfort him. Then, it hit him like a brick wall, the sensation almost winding him as it crawled its way inside of Astarion’s so very empty stomach.
The scent of blood. Fresh blood.
“Oh, gods.” Astarion practically purred.
Gale breathed a chuckle, expecting this reaction at this point. “I know, I know. My apologies, I did not expect my cycle for another week or so, but fate had other cruel ideas. The pain has been keeping me awake.”
Astarion licked his teeth from under his lips, his throat suddenly feeling incredibly dry.

"Gale's body is changing, his hormones shifting, and the effects are becoming clearer in him. He's softer, rounder, and though there has been no visible change to his abdomen thus far, Astarion cannot help but press his hands into it and imagine their child growing there. An impossibility, a scenario beyond his wildest fantasies, and yet it's real."
An unexpected pregnancy does not mean it is an unwanted one. Gale and Astarion find this out for themselves and try to cope with impending parenthood (by having a lot of sex, mostly).
